Extension strange behavior

Hi;
I have noticed strange behavior in my elastix system (asterisk 1.8.15)
From time to time, users are not able to establish connection from one extension to another one. They get busy signal, but the called phone has no connection in this time.

It's look like the called extension have established connection with his own extension during this behavior appears.

has anybody seen something like this and could explain what does it mean? ow to fix this?
