Grandstream 4004 and faxing

I am trying to get the grandstream 4004 setup to fax per this article http://www.3cx.com/blog/voip-howto/faxing-gxw-4004-4008-4024/

I seem to have everything setup right but it wont provision. I cant see it in the pstn devices.

One thing interesting is once i go back into the setup of the 4004 the field where i enter the provisioning server is blank
I enter 192.168.1.10:5000/provisioning
If i hit save its there, but once i reboot and go back its blank

one thing interesting is it look like it did kind of provision because it has my fax extensions if i go into the fxs ports tab on the gateway web config

well one thing i see when looking at my log is that i am getting an error on my 3cx that the credentials dont match.
I dont see where to add the password on the grandstream though.
