How are you provisioned? STUN, SBC or local?

Is the extension blocking STUN registrations?

Are you sure your credentials are okay? Also, make sure that each account has a different SIP port (in SIP or Network Settings, under the Account) since you're using STUN.

What is the goal for the setup here?

Hello
i looked into more for changing port but it will be changed for all accounts and account 1 is up and running without issue

What firmware is on the base?

I would recommend you open a ticket on Grandstream Helpdesk and ask them for the DP75x template. They have one. It's an FXS you need to upload/move manually but it works well. They will explain the steps.

The template is made to be exactly like you want: 1 extension per handset.
