Looks like it may have to do with the csv file.
If you are editing it using Microsoft Excel make sure that its preserving it in the original format and not converting it in any way.
 
If you are unsure if MS Excel is somehow changing the format of the .csv file, use a text editor to edit it, even if just as a test, then try importing the new one.

If that one goes OK, you will at least know that Excel is doing something.
 
Make sure you don't have extra columns. you will see this in notepad as commas at the end of the line.
Also I always assign ID numbers to the rules in an arbitrary range (eg 500,501) above the current rules. 3cx will change them but they seem to need to be in the file anyway.
