With a 3CX hosted system, are there any issues intercoming from a phone in location A to another phone in location B?
Also, is there a way to have one of the BLF buttons be a direct intercom button to a specific extension?

The answer here is that there should be no issue with this as long as the phones are supported and provisioned behind 3CX SBCs.

You can add a custom speed dial BLF key to intercom an extesnion directly. Just ensure that the rights of the users are set to a role with the option 'Can intercom' enabled and that under the Management console > Settings > Dial code, you have a dial code for the paging section, for example, *11.

Then you custom speed dial BLF will be for the number *11EXTNumber. (i.e., *11000 to intercom ext 000)
