Invalid PIN on conference call

I am receiving reports of seemingly random failed PINs, even though the PINs are correctly entered.
This happens both internally and externally.

There's no errors on the server side, not even sure how to debug this...

Please advise, it's a pretty big issue.

I thought so as well, but it's sporadic and also happens locally, digital receptionist works.
