provisioned how?

try putting them on the same lan as the PBX - if issue persists then you've got a routing issue.

but it works when i restart the phones or just make any outgoing to any extension. then it works for sometime. again the same. if there is any routing issue it wont work all the time. i have checked all the routing rules. IP Phones were provisioned manually. using the SIP credentials

you'll have to do some network captures and reproduce the issue to see what's going where.

Almost for sure you are using STUN and the sessions / states are closing after some time. Which is why it works after reboot for a short amount of time.

Use a SBC if no direct routing between PBX and phones.

Sir, these IP Phones are in the same office only. but phones are in the different VLAN.
3CX Server in 192.168.0.0/24 , IP Phones in 192.168.100.0/24. i cant use SBC or STUN for provisioning why because when provisioned in STUN or SBC, when internet go down all phones will not work so used manual provisioning.
please help me in this

Sorry, I should not have said STUN here, but rather DirectSIP. Regardless, it doesn't change what's happening.

So I understand the phones and the PBX are in different subnets - how are they routed to each other? What device is making the VLANs talk to each other? It's that device that would appear to be closing connections so that the PBX can't send packets to the phones.

If I had to guess you have a firewall doing the inter-vlan routing and that will need to be adjusted. But maybe you have something else.
