OK, this is my personal experience, so take it as "be aware":
Bottom line, on my iOS devices (iOS 15) whenever NordVPN is active, connections, movies, netflix, browsing, making calls, AppStore, TestFlight, blablabla, all are working like a charm (NordVPN has good servers) but everything what's incoming via standard and/or VoIP push doesn't reach the device - it's very obvious in case of standard push: suspend VPN and suddenly you get a burst of notifications from Mail, GMail, TestFlight, news, 3CX chat/missed calls, WhatsApp and so on. Based on my/our experience with push, this is a clear symptom that Apple APNS server sees the device as being offline, so it caches the standard pushes until device appears "online" again to be delivered.
So just keep an eye in case you don't receive push notifications on your device when you expect them and check if you're on a VPN.
