Office 365 integration instructions.

I would like to point out that the instructions in Step 1 of the Office 365 Integration on 3CX entitled Configure App ID are soon to be incorrect. In May 2019, Microsoft is requiring that App IDs are created in the Azure Portal. It would be nice to get the Azure Portal instructions.
