One computer/user unable to login to MyPhone

I have phone system pro version 11 with 12 users. All of my users are able to login to Myphone successfully except for one. When she visits the /Myphone page the application loads and she is prompted to enter her extension and pin. Though her login information is accepted it seems to be stuck in a loop and the login screen pops back up over and over again without logging in. This only occurs on her computer and she is able to login to Myphone on other computers. I have tried removing the Myphone app installed locally as well as uninstalling and re-installing Silverlight. I have also tried clearing the browser cache and internet history. Nothing works. Any ideas as to what I can try to do to resolve this

I have this in the system event log:

Can this machine log in to some other extension number besides 101?

No we cannot log into any other extensions on this machine.

Obviously something wrong on the machine in question. Probably quickest/simplest way is the great granddaddy of solution:
