I am slowly learning dial plan syntax. The above was the default string which I appended by adding the part before the first vertical hash mark. As far as I can determine, this string seems to say "you can dial pretty much anything inside the USA".
I can see a couple of issues right off the bat. If you choose not to use an access digit for outside calls, then i would recommend you not use extensions that begin with one. As it stands, you would never be able to dial some long distance calls as there could be a conflict between an extension number and the beginning of a long distance number. You also need to include other 3 digit 3CX numbers, like voicemail access, etc. The dialplan is read from left to right, so keep that in mind when "placing" new additions.
The [] brackets are only necessary if you are including more than one digit, or a range.
