Solved Outbound Caller ID issue

I have one extension that we'll call "100" that has a caller ID that shows up as Sri Lanka and all other extensions we'll say are "101 through 120" show up as a US local number. All extensions are on the same network so no remote extensions are being used. All extentions/DIDs use the same Outbound Rule. Caller ID on "100" is identical to "101 through 120".

Any thoughts?

If all extensions use the same trunk, and the same outbound rule and the same outbound caller ID, then check if there is any Caller ID reformatting on the trunk. You could also run a wireshark capture and make 2 test calls. One from an extension that is showing the correct outbound caller ID and one from extension 100. Compare what the PBX is sending to the provider. If the caller ID sent is identical then contact your provider.
 
Also, confirm the extension doesn't have an explicitly configured caller ID.
 
Turns out that the length set on the outbound rule was set 7,10-11 and was stripping numbers incorrectly causing the number to go out over the SIP as +94 (0) (XX) XXX XX. Because it started as +94 (which is dialing code in Sri Lanka) it appeared as a call from that country. I have since changed the length to simply 10,11 and the problem is resolved.
