Phone provisioning issues

I think this started happening recently, I don't have an exact time frame. I'm trying to auto provision a Yealink T42G, which I have done in the past without issue. I've tried doing it from the phone's web interface but nothing happens, so I tried to capture some logs.

In the logs, I can see that a request was made to the correct server and the directory looks accurate. But it immediately fails with a 404 error. I'm not sure what exactly it isn't finding.

If it helps to know, I'm running 3CX 15 SP2.

Not for that particular phone as it's not in the console yet (since it won't pull down a provisioning template). However, out of curiosity, I did the exact same thing with another phone that was already in the console and I immediately got a "Not Found (404)" error.

I'll investigate my templates folder on the server to make sure nothing is missing or corrupt.

Yeah, try with a fresh factory template. I had the same issue with a custom template and found I'd missed an ENDIF. The +Config button is an easy way to see what the phone will see - if it's blank/404, then it's much more likely to be a template issue. I can post one here (or you could probably PM one of the 3CX guys if you aren't comfortable nabbing one from a stranger).
