Remote Phones behind pfSense, PBX on Azure, Reg and Audio Issues

Prior to V16 Update 5 (16.0.5.611), this site had no issues. No changes were made.

PBX is hosted on Microsoft Azure, Linux Debian 9.12
Remote Phones are behind pfSense 2.4.5, Firewall Opt-Conservative
3 Yealink T48G, 1 Yealink W52P DECT, 1 Grandstream HT802
No SBC, using STUN
No Template Changes
Firewall Check passes

After V16 Update 5, phones experienced odd issues, like directories and queue issues, and would fail every 24-25 hours.

Logged into the Yealink phones admin page to see "Registration Failed". Any attempt to re-register continued to fail. Verified no connectivity issues. Reboot the phone, and it registers and works without issue for another 24-25 hours. Resetting the firewall states also works too.

Factory reset phones, reprovisioned. Directory and queue issues are gone, but now phones maintain their registration, but at a random time point there's no audio in either direction.

I realize everything points to a firewall issue, but other locations with the same firewall and phones aren't having these issues. I pointed 2 Yealink T48S and 1 W52 phone (at my location) to this PBX instance, and have had no issues with calls.

Anyone have any thoughts?
