Ring Tone Changed

I have several clients where we have on premise 3CX systems. Over the past couple days we have gotten notifications that the default ring tone has changed at several locations. The default ringtone for the phone is System Ring Tone and the template for the phones they are using show the first entry as the System Ring Tone but at each of these locations, the ringtone for every phone has changed to Ring 3. Any suggestions would be greatly appreciated.
