So I have a situation where when a spam calls come in the caller ID of one of our active users is attached to the number. I have no idea what could be causing this. Also, our receptionist is first in line when it comes to inbound rules so that might have something to do with it. Any advice?

Spam callers are known for using someone else's valid caller ID as their own, however, it is a bit of a coincidence that they are using your number. 3CX does have the ability to add a name, in several places, and a number can be used in that field. I would check the 3CX Activity Log, to see if the number was originally sent by your provider. It may also be that this is supposed to be a DID number, and somehow it is being displayed as caller ID instead.

I'll use the name Jane Doe to refer to the receptionist. I found an instance of this happening in the call logs. Its an external number with the caller ID " :Jane Doe (+1 xxx-xxx-xxxx)" in the "from" field and Jane Doe (*ext*) in the "to" feild. The number that follows the caller ID is not one of our numbers.

A name can also be added from the company directory, if a number match is made. If it is being sent by your provider, then there is not much you will be able to do, but worth a talk with them.

And to add to what @leejor said, in can also be added from the Inbound Rule name that is being used.
This means that if you have and Inbound Rule for DID "+1 xxx-xxx-xxxx" and it's name is "Jane Doe", regardless who calls that number, spamm caller or not, "Jane Doe" would appear on the device.

[EDIT]
If you want to remove this "prepending" of the Inbound Rule name, in the Management Console go to Advanced --> Contacts, here press Options and select "Do not add" in the "Add Group, Queue or DID/DDI Names to Caller ID" section.
