Had a strange issue today. Had a brand new T46S that was running the latest firmware through 3CX. It was working fine. Factory reset it and couldn't get it to fully provision again. It would get the extension number and i could call in and out to that phone but the BLF keys would never come back.

I deleted the extension.. factory reset again.. resetup everything again.. Nothing.

Ended up manually upgrading the firmware with firmware from Yealink and then everything worked fine. Although 3CX tells me its unsupported.

Is this something typical?

Firmware that wasnt working right: 66.86.05
FIrmware I upgraded to: 66.86.15
 
You haven't mentioned, but it sounds like you are using your T46S in STUN mode.

If that is true, the firmware upgrade is not the reason you are facing that issue

Negative. Some are remote using SBC.. some are local on LAN.

Ok so far it sounds good, but this does not tell us what might be wrong just yet, lets look further.

Usually we see BLF issues when there is time drift on the server, and causes BLF to become invalidated or the machine is not specced correctly and cannot keep up with requests (BLF timeout).

1. Does the PBX machine have enough resources for your system size?
2. Does the SBC machine have enough resources for your system size?
3. Does the SBC fall within the total number of BLFs / extensions recommended or is it overloaded?
Check the specs and compare them with this to ensure you are within the recommended minimum specs:

3. Try installing NTP on your server, and then reboot the PBX first, and then the phones once the PBX is back.
We should see improvement if this was due to VM time drift:
Bash:
sudo apt update

sudo apt install ntp
Note: It will not help at all if the machine is overloaded, time is the least of your worries in this case.
