Transfer call gets disconnected

Dear all,

At random times a day someone calls our company. The receptionist picks up the phone and starts talking. Then the call need to be transferred to another internal number. So the receptionist puts the call on hold and starts dialing the another internal number. The other number picks up and the receptionist tells that she has a call for him. Then the receptionist simply end the conversation by pushing the red button. The call should be transferred to the other internal number but in stead the call gets disconnected.

Anyone has any idea about what is causing this?

Can the phone she is trying to transfer the call to reachable at all?
Can you call that extension and confirm that it's working?

What make is the phone and what build is the phonesystem you are using.

We are facing the exact same problem.
I'm so tired of it that I'm thinking about moving back to Trixbox.

We use Aastra 9480i phones
We run 3CX v.9
Our VSP is iristel, authentication is done with IP address only and works well.

Every now and then our PBX can't transfer calls.
First person that answered the calls looses ability to pick-up the recently transfered call.
Person to whom the call was transfered can't pick-up the call either.
Caller is on hold and listens to "onhold.mp3" until he gets tired and hangs up.
a restart of the PBX services isn't sufficient, I need to restore a backup to get the transfers to work again.
