Transfer to number initiates 2 calls

Greetings, new here.
I have an issue when an inbound call to an extension is forwarded to a number (outside number) it initiates 2 calls.
Example: Calling from cell phone to 3CX DID -> Forwards to Skype for Business DID -> shows 2 inbound calls from the cellphone.
I am at a loss as to where to look for correction, and any assistance appreciated.

I wonder if you have your extension setup to "ring my mobile simultaneously", and at the same time have the 3CX Phone installed? If that's the case, perhaps it is ringing you cell phone, and 3CX Phone at the same time?
 
Title should have been "Forward to number..."
On the extension setting, the "mobile number" is set for my skype for business DID, and forward rules all set to "forward to mobile".
Under options "Ring Mobile Simultaneously" is not checked.
I have also tried "Forward to Number" and entered the skype DID- same results.

So which is it...2 calls are initiated (outbound?) upon arrival of an inbound call, or, it shows two inbound calls, then forwarded?

Could you post the 3CX Activity Log for one of these calls?

Clarification- One call into 3CX DID assigned to extension. Extension set to forward calls for all conditions to external number. External number receives 2 calls simultaneously from 3CX when DID called.
 
I know it's a lot, but here is the log: {phone numbers and IP Addresses redacted for security)

Wireless 1847xxxxxxx is the caller
603xxxxxxx is the DID
6550 is the extension assigned to the DID
6355 is the extension transferred to
1708xxxxxxx is the external number extension forwards to

So, you're also using Bandwidth.com. Did you read the link I posted previously?

Same issue, same provider.

Perhaps the same solution.

Thanks, that is what I meant... server change, not host change.
I'll have to wait until a down time to do so.
