Upgrade to Version 20.0 Update 10 (Build 1621 Alpha - AI 1.6.57) with run away AI auto attendant

We upgraded our in-house, on-premises 3CX system to Version 20.0 Update 10 Alpha 2 (Build 1621 – AI 1.6.57) this morning. Before the upgrade, we were successfully using two OpenAI AI Auto Attendants and OpenAI transcription services.

Approximately two hours after the upgrade, the AI Bridge began generating a continuous flood of OpenAI requests and 3CX Event Log notifications. The Event Log accumulated approximately 61 pages of Event ID 4096 errors within a very short period.

The repeated errors included:

“Rate limit reached for gpt-5.6-terra… Limit 1,000,000 tokens per minute (TPM), Used 1,000,000…”

and:

“SQLiteSession is closed”

The OpenAI Usage dashboard recorded 1,154 requests and approximately 5.55 million tokens for September 15, with a cost of $2.57. This level of activity was far beyond our normal call and transcription usage.

To stop the repeated requests and notifications, we removed both AI Auto Attendants from the 3CX system. The activity appears to have stopped after they were removed. We have not recreated them because we do not want the runaway request loop to restart.

Please confirm:

  1. Is this a known issue with Update 10 Alpha 2, Build 1621, or AI component 1.6.57?
  2. Is the “SQLiteSession is closed” error related to the repeated OpenAI requests?
  3. Is there a supported procedure to repair or reinstall the AI component without affecting the rest of the PBX?
  4. Is it safe to recreate the AI Auto Attendants, or should we wait for a corrected build?
  5. Are there any logs or support-information files you would like us to submit?
The production telephone system remains operational, but the AI Auto Attendants are currently removed and unavailable.
