Is there a way with voice prompts and someone selecting an option and that option showing up on an extension (IE: someone pressed 1, 2, 3 etc.) and it shows up on
the app extension or physical phone extension?

3CX does not log what option the user selected, nor is there a way to have that selection display on the phone. The closest you could come is to write a custom Call Flow Designer application that logs the user's selection, and a separate client application that pops up a dialog displaying this information.

To do what @VoIPTools mentioned however, you would need to have a dedicated instance of 3CX, not an SMB instance.

Absolutely correct @NicholasP_3CX. I missed that this was a SMB specific question. Thanks for making the correction. That being said, I always recommend purchasing the Pro edition.
