VOIP extension utilizing old PTSN PBX

I have an old AVAYA v3.0 PBX with 16 wired analog phones in the office. There is no attendant installed. All phones are ringing at the same time. I need to add just one extension for a remote user via the Internet (in a different state). High speed Internet is also available on both sides. Do I have to replace the whole system for it to be IP capable or is there a solution for connecting one of my phones via the gateway?
 
You could set up a simple scenario where you have one trunk (an FXO ports on something like a SPA3102) connected to an extension off of your current board and at the remote end a single line (FXS port an an ATA). It could be accomplished in other ways without using 3CX in the middle but unless you are familiar with VoIP devices, this would probably be the easiest to set-up and troubleshoot as you could get each portion , line and trunk up at a time.
