Unsupported Yealink - Multiple lines on a T46U outgoing calls use wrong extension

We have this setup on fanvils without any issue but have the issues on Yealink.

There are 3 extensions per phone, the phone itself 500 and two extensions for voicemail 501 and 502, calling is disabled for 501 and 502
The phone is assigned to ext500 and was provisioned through 3cx, the other two lines were added via the web interface in account slots 2(ext501) and 3(ext502)2.
Normal operation is you just start dialing the number you want to call and it calls out correctly from extension 500

I am aware we can set the line key up for the phones extension and they can use that method but I am looking for a resolution not a work around.

At the time of initial provisioning everything works as intended, outgoing calls call out via the 500 extension, and the voicemails for 501 and 502 through the menu phones menu without issue.
After a while it starts acting up and starts using 502 for outgoing calls, as 501/502 are disabled it gives a forbidden error on the phone itself and the 3cx call log shows correctly as External Call disabled.

I am looking for the setting in the phone or 3cx template that periodically that seems to make it use the highest extension
