When I try to register my T40p to a remote 3cx in the cloud. It registers with a basic ID, example, extension 104 registers when I set the Register Name and User Name on the Yealink to "104" with the 3cx generated complicated password, EfRCq1SgrL.

However, if I try to register the Yealink to the 3cx, with the 3cx generated Register Name and User Name, ID: 5roKeT3VXE - The phone refuses to register, even after a reboot.

This same thing happens whether you put it on "disallow use outside the LAN".

What could be the cause for this?

Registers:
13469

Refuses:
13470

In Your refused settings the user name should be the extension number 104. Only the register name needs the complex user ID. You should use auto provisioning it will remove these sort of errors.
