Yealink T42S answers using speaker phone

Good morning,

I have a client that is using a T42S, firmware 66.84.0.95 (up-to date), that will occasionally auto default to answering on speakerphone.
We have 3cx v 16.0.4.493 installed on our PBX. The phone is provisioned via local lan, and no other phones in the company are having this issue as far as I am aware.
We use ATT as our SIP provider. The firewall checker passes all but SIP ALG
All phones are also part of the same call queue

I have disabled "Accept Multiple Calls" on this extension, and I have changed the "Phone Provisioning" for the 3cx app to default to the IP Phone.

Since the speaker function works, I know the phone is working, but I cannot find a reason that would cause the phone to answer every 5 or so calls on the speaker phone. The end user states it happens most often when a client calls in on a cell phone. Any help would be appreciated.

Do you mean the phone auto-answers and goes to speaker phone or do you answer the call on the 3CX client and the call is answered on speaker phone?
Did you try to factory reset and re-provision the phone to see if that helps?
 
When using the IP phone to answer it will use the speaker phone in the base instead of the handset. They are answering by picking up the handset.

I have not factory reset the phone, but I have rebooted and reprovisioned.
 
Try a factory reset and see if that helps. If the issue persists and this issue is with a single handset then i believe you may have a faulty handset.
To eliminate the PBX configuration you could swap it for another phone and see if the new phone has the same issue or manually assign it to the extension and see if you can replicate the behaviour.
