Solved Yealink T58-A: Credentials change on Auto-Provision?

Hello all!
I hooked up my Yealink phone and it got a local IP address. I logged in with the default admin/admin credentials.
I then Auto-Provision the phone with the Auto-Provision URL from the dashboard.

Once Auto-Provisioning completes, I can no longer log into the phone!
admin/admin no longer works.

Can you please let me know what credentials I should use? I even tried the User Name/PW under that particular User (Extension), but that did not work either.
This is in "Authentication details used by phones & apps. Reprovision after a change".

When a phone gets provisioned the login credentials are changed based on the configuration of the extension.
The username remains admin and the password can be found on the phone provisioning tab of the extension in the field "Phone Web Page Password".

One more item...is there a way to use a custom Date Format.
I would like to have: WWW MM DD YYYY (Mon Jan 15 2017)

The only available formats are what is listed on the phone provisioning tab of the extension.
