I have only tried this scenario inside the LAN where the 3CX server is situated.

If I turn off wifi on my iPhone and then go to the extension on the 3CX management page and then scan the the QR code, nothing happens. If I turn wifi back on and scan the same code, the phone provisions.

If I turn off wifi on the iPhone but then scan the QR code from the attachment sent in the welcome email, the phone provisions.

Any ideas on this, please?

The QR code on the Webclient and Management Console are generated from the URL you're using to access it. Meaning, if local (http), it will give a provisioning link local only, which doesn't work over LTE/3G. If you use HTTPS with the FQDN to access the page, it would give you a working QR code.

The QR code in the email is always generated using WAN IP/FQDN so it can be accessed from outside the network.

Fantastic, I suspected there may be an easy answer. Thank you.

Correct, the QR is context sensitive when in the management console, but the one in the email will include the FQDN always to allow for remote users to provision
