40% failure rate for updates

I manage 5 3CX phone systems, this weekend I upgraded them to the current version. I've had to completely reinstall 2 of the 5 after the upgrade failed and made logging into the management console impossible. why is there such a high failure rate for this update?
 
Are these Windows servers or Linux boxes?
 
No idea but seems like it might be something specific to your setup, otherwise I think we'd see a lot more complaints here. Care to share more information? Windows or Linux? What version to what version? Hardware, virtual, cloud? Were phone services working still?
 
One each, Linux box was virtual, Windows was bare metal. Both were upgrades from the previous version. 16.4 to .5 neither setup is much more than basic install with phones set up. I’ve not needed to have any overly complex setups in my systems. Normally I would have rolled back the Linux VM but my employee forgot to create a checkpoint prior to upgrade.

Steps to upgrade were make a backup of phone system and then install update from the management screen. After update complete email we tried to log in to the management console and received a server error message on the login page.

Rebooting the systems did nothing to resolve and both required a reinstall to regain access.

In the upgrade email, can you check if it said the it completed successfully or that there was a failure?
Did you check to see if all services were running after rebooting?
Do you have any other software running on the machines at all?
 
Both said successful
Yes all services were running on both machines
No other software on either machine. Both are dedicated phone systems.

Were you able to see any error messages when trying to log into the MC or did you perform any additional troubleshooting that would give some indication or direction to investigate further?
 
I did not spend any more time than needed. Getting the phone system up and working was paramount so I reinstalled within a few minutes of troubleshooting.
 
Understood, in case you face this with a system that is not critical (ie. a testbed system) on which you can afford downtime, we would be interested in taking a look
