Are you using a 3CX FQDN and are both systems on-prem?

Are the two PBX's both remote and with two different public IPs?

Was the DNS and A-record configured to solve this issue? The fact that you mention configuring a DNS leads me to believe that the Passive server is configured to use the FQDN of the Active server. If that's the case, I'd recommend using the Active server's IP address in the "IP address of ACTIVE server" field as per our guide: https://www.3cx.com/docs/failover/

Also, what options have you set here and what failed on the Active server the last couple of times the failover failed:

Depending on the situation, the failover may not trigger based on these settings.

yes two different public IPs as well!, using the active server IP in the Active field are you talking about in the failover option?

Here are the failover setting.I'm confuse to where you mention the active IP of the active server.

Sorry as I may have not been clear. I meant the Failover settings on the Passive server FOR the Active server.

ok thanks yes thats set to the Active server. Should it be the public IP or the local IP address of the active server right now its the local IP.

If the two servers are on the same LAN then it should be the Local IP Address. If the two servers are on different networks and the only way to reach each other is via the Internet, then it should be the Public IP.
