Solved Bulk Import failing with "item of same key has already been added"

I'm trying to import around 90 new extensions into our instance and its failing with the error "An item with the same key has already been added. Key: cisco79XX.ph.xml_Cisco 7940"

This is a Debian instance running the last update to v16 (too many things changed in the web client UI to push them to 18 without a LOT of training).

I have checked and double-checked my import sheet as well as dumping all current extensions via export and the string "cisco79XX.ph.xml_Cisco 7940" doesn't show up in either file. Likewise there is no template named this in our system when I view the templates under settings. I'm really at a loss for where this is coming from and really need to get these extensions imported. Thoughts?
 
If you split the file, and try importing, let's say, just the first line, do you get the same error?
 
No even a single extension/phone import breaks with the same error. I'm unsure where this is breaking down
 
Could you upload a single line that you are trying to import and fails here?
 
Sure, here you go. This file was started by just exporting an existing extension, deleting that row and adding in the values of the new extension.

The only thing I had to do was:
- Rename file to .csv
- Make the extension a 3-digit extension instead of 4 (because I only had a 3-digit PBX right now)
- Remove all the other lines
and it worked just fine for me:

So I am not sure what you are doing wrong. I tested on V16 Update 8.
Based on the error, have you been doing any tinkering with the IP Phone templates? I couldn't get what, but that seems like it's referring to IP Phones.

I think only one phone has a custom template and its a yealink phone and the template is just adding a button item that the extensions BLF page doesn't have.

They no longer use any cisco phones and there aren't any set up in the system. When we first deployed 3CX years ago (i think this was in the v14 days) we tested with some Cisco phones and as such there are some old unused custom templates for them but none matching the one listed in the error. I'm going to go ahead and delete them and see if that helps
 
Well having deleted all of the cisco custom templates I'm now getting a new error "An item with the same key has already been added. Key: yealinkT4x.ph.xml_Yealink T19P E2" so I'm at least making progress. I'll clean up as much of this as I can and report back

Found the problem. There was somehow a custom template with the same filename as the default yealink template. Getting rid of that resolved the issue.

I don't seem to recall having ran into this before, but does make sense because the system wouldn't know which file of the 2 to pick.
