Busy On 3CX ignored

When there's a call on the 3cx app and multiple calls is off on the mobile, a second call on the mobile has no busy and pushed the 3cx app to the background. Notifications are given (sound & screen).

It looks like the Busy is ignored by the 3cx app

iPhone 5SE, IOS 12.0

Sorry, I don't understand what do you mean by "Busy"? iPhone's device DND? Or 3CX client status? Or a 3CX client SIP busy (486)?
