Call answered by two parties, in two locations - one could hear the other

This has never happened before, and unfortunately logging was set to 'low', so I can't do much to diagnose the cause. Perhaps someone else has experienced similar?

The setup:
PBX is hosted at datacentre. Office has SBC with hard phones. Remote user (me) has iPhone app.

What happened:
Incoming call came in, rang ring group which included office user and remote user. Both answered at the same time. Remote user could hear both sides of conversation and so hung up. When asked, office user could not hear remote user answering call and was unaware. Call progressed normally and office user was able to transfer to another office user on same SBC.

Has anyone experienced this before?

Without logs or a capture there is really no way to know what happened but in theory this could occur if both agents answered at the same time and one of them was using a 3CX client with Push.
When you answer a call from a client with Push the app needs to unlock, register to the PBX and receive the call.
This creates is a small delay that could (in theory) allow another agent to answer the same call before the Cancel from the PBX could be sent to the other agents.
Again, not sure if this is what actually happened but it is a theory that could apply to the scenario you are describing.
