Calls dropping on transfer

Issue with transferring calls. Have PBX in the cloud and SBC onsite.

Seems to be happening a lot.

click on the BLF button
speaking to them and then put handset down but it doesn’t make it through to him.
It shows at his end as if he’s still talking to me and we lose the caller or they go straight through to voicemail

Phones are all T46S.

Any ideas?
