Calls keep being terminated

HI all,
Have a problems with some DID, the calls keep saying "End destination cannot be reached" but the extension is on and i cant find a rule where it would be stopped.
not in the call handling or the trunk it self. any ideas where else can i check?

If you run a call report what does it show?
 
I have this exact thing happening as well. The Call Report on my end shows (edited for privacy):

And the user has called 6 times in the last few weeks with the exact same result.

Negative, there are no CID Rules nor CID Blacklist settings for the caller's number.
 
Does the specific user the DID is for have any exceptions? Terminated by rule means that it is matching something.
 
Both CID Rules and CID Blacklist are currently empty with zero rules in place. I also just checked and the number they're calling goes to an IVR, which does not have any exceptions to the rule, either.
 
How is the DID assigned, and how is the provider sending the DID in?

If your DID is in the format of +12525551234 and the provider is sending it as 12525551234 then that would mean the PBX is not detecting it as a valid DID and is sending it to your trunks default route. What does that have assigned?

Check that this is not defined as End Call.

An easy way to check what is being sent by the provider is to do a capture on the call and see the format of the DID in the TO field of the INVITE coming in.
