Audio for all calls that are forwarded/transferred works fine. However, any immediately received/made calls only have one-way audio: a caller/callee to one of our softphones cannot hear any sound from the softphone making/receiving the call (but the softphone user can hear the voice of the other party).

As far as I can see the log doesn't point out any difference between the call being made to the original extension and the 'forwarded version' of the call:

Are the "Supports Replace" and Supports Re-Invite" option in the Provider Settings -> "Advanced" unchecked? Also PBX Delivers Audio there should be checked.

If that is OK, then this will probably be easier to troubleshoot using Wireshark to see exactly what is happening...
