I've tried many different entries in the CallerName field and nothing seems to show the name on inbound calls.

I called Babytel and they aren't super helpful either.

I'm completely lost as how to proceed with getting this resolved. I was looking in event logs earlier but can't make sense of them.

In the SIP trunk parameters, go to "inbound parameters" and make sure "CallerName" callers name is set to "From: Display Name".

I have, still nothing.

I know the CNAME is being sent with the call (test call is also coming from Babytel btw) because when I call my personal cellphone on Koodo, I get the CallerID Name showing up.

Are you referring to outbound caller id or inbound caller id?

Or are you trying to retain incoming caller id on a call that goes to the pbx and then transfers out to a cell phone?

Inbound callerID name.

When I receive a call, I only get the number, not the name.

However, the phone I am using to test this ALSO uses Babytel as the SIP trunk. I was mentioning that when I call my personal cell phone number, I see the callerID (name AND number), so I know the test phone I am using to place the call is sending CNAME with it.

I'm simply calling one of my extensions in 3CX (which uses a Babytel SIP trunk) from my work phone (which uses a different Babytel SIP trunk) and receiving the call on the 3CX app on my iPhone.

It only shows phone number.

You should do a packet capture and see what header is being used to convey the Cnam. From there you can adjust the parameters accordingly.

Most providers don't allow you to send a name. There is usually an addition charge for caller name display on incoming calls, so unless you subscribe to it, you may only be receiving a number. When you do see a name, such as on your mobile phone, it may be because a match is made in the phones directory.

I will agree with @lneblett on this one. You need to see what SIP field Babytel is using to send the cnam value. Then you can adjust the PBX accordingly to read from that value.

Our BabyTel trunk for incoming cnam uses: From: Display Name.
We also have our outbound From: Display Name set to custom and our company name is sent.

Also be careful when using your cell phone as a test. If the caller id number is a contact on your cell phone, it will use the contact name on your cell phone.

If you haven't already, I think Babytel requires you to sign a release form for third party cnam. The specific form is called "Specific terms of use - Extended Caller-ID functionality". After this is submitted, they will enable a setting on your account to respect the outbound cnam from your pbx.
