Can't Add Phone

Using most current version, 16.0.655, trying to add a Yealink T21P E2, which is supported, using current firmware version. Every time I go thru the add phone sequence on the console, the phone never shows up in the list of phones. If I add the phone via the extension page, it seems to work but will not register and does not show up on the phone page.

On the phone I get a screen message that its updating the configuration, then one that says update skipped.

I restarted the services from the console, I tried rebooting the server, tried resetting the phone to factory defaults.

Since its not on the phones page I can't try and edit or fix anything from there.

How to I get it to show up as a phone ?

Additional info -
Running on Debian, hosted at Digital Ocean, password checker passed ok, set for STUN, disallow option cleared. All other phones working, using Fanvil on other extensions plus android app and softphones working. No problem adding new softphone, but when I did it showed up twice in phone list 1 with my ip and once with 127.0.0.1.

Update, finally got it to register, not sure what did it, reset it to factory, reload firmware etc. Now I have 2 entries for the phone, but it works. Not sure how to get rid of the duplicate. Only 1 account on phone enabled.

Nice. Usually when you have two entries for a STUN phone that implies some sort of SIP ALG or other weirdness on the firewall at the phone side of things
