Cisco ATA 186 with External 3cx system

we are using an external 3cx system and has been working great. now i'm trying to get a cisco ata 186 unit to work with an analog phone.

here's the settings i am using, where 4200 is the extension number.

any thoughts on what to do to get it connected?
i don't see it getting to the 3cx system..
