Current call drops when new call comes in

I'm kinda new to 3CX, but I'm working with someone who seems to know what they are doing. But we have a problem that has us stumped. Our users are complaining that, sometimes, when a new call comes in the existing call gets disconnected. We cannot find anything in the logs, or a problem with the phones.

Anyone have any idea, or a good point to start from?
 
Some logs? Server activity log, or wireshark, try to simulate that while wireshark is started? Are you using sip trunk from internet voip provider, PSTN or something else?
 
What brand and model phones are you using? What firmware? What is the configuration of the incoming call?

We have an intermittent problem at a site running ver 11 with Yealink T28's firmware version 2.61.0.99, with 3 phones in a ring group. Occasionally with one phone on a call a second call comes in and cuts off the audio of the first call when the second call is answered by another phone. The phones and ring group are set to allow multiple calls. 3CX still thinks the cutoff call is in progress and the original caller just gets silence. Multiple wireshark capture attempts have not yet shown a definitive reason for the audio cutoff.
 
We are running Yealink t28. The call configuration is the following: Incoming Call -> Extension 300, Office Operator -> Office Open/Office Close
Office Close: -> Voice Mail
Office Open: -> Ring Group 1 (Contains the extensions)
