When changing internal DNS servers (IE: Old DNS was 192.168.1.10.. New One is going to be 192.168.1.6), is there anything i need to change other than the DNS server address hard coded in the appliance?
 
That should be it, you would change the DNS on the NIC of the PBX.

thats what i thought too, but every time we kill the dns service on the old server you cant make outgoing calls
incoming seem to work fine
 
That's unusual, as long as you have the appropriate forwarders set up on dns outbound they should be able to resolve to the voip provider and connect the calls.

is 3CX set up using split dns or different external / internal DNS?

what happens when you make a call does it attempt to connect, is there anything in the activity logs to suggest why the call is failing?

It attempts the call then drops it with this error: [CM503003]: Call(C:1177): Call to <sip:[email protected]:5060> has failed; Cause: 503 No DNS results/INVITE from local

I didn't set it up originally but im quite sure split was not setup.
Going thru the 3cx settings i cant see anything that references a DNS server at all, let alone the old one
