Echo on Internal Calls

I have bad echo on internal calls no matter what device I use, the echo is clear word for word is echoed back.

Setup is home 3CX server on version 18 on VMware with firewall rules to allow calls in and out. If I'm on my home network or call through SIP PSTN I have no echo with other devices. If I call an off network extension through the internet is when I experience the echo. Feels like I am internally bouncing back somehow. I can not tell if my device is using my internal IP or external DNS to connect the device.

Just did some testing on a device connected to the internet and i do not seem to have an echo. could it be just the one extension I call with the issue? Thanks!
 
Just had the extension delete the app from his android phone and reinstall it and we still have the echo.

Echo is going to happen when there is a delay in audio, which is the case many times with SIP, and the senders audio returns. It is less likely to happen between devices on the local LAN, but some network hardware could be causing it. You might need to do a process of elimination.

Most devices are able to overcome this (many do have echo suppression options). If the device experiencing the echo, is sending audio at higher than expected level, then the other end may not be able to suppress it, and it "leaks" back.

I would also check the Codecs being used. Some low-bit-rate Codecs require more processing power, and on some devices, can cause longer than "normal" delays. If you are not, try using G711 a or U, as the default Codec.

You say it is happening on calls to off-net extensions. Is this just at one remote location? Have you done any network tests, at the far end.
