Hi,

on activity log i see sometime this message:

Hi,

What specific version of the PBX are you using?

This is just an error that is generated when, in one example, the destination could not be reached for some reason (depending on the case).
 
Windows version v16
 
phone on system have PIN protect enable, is possible this error occour when user don't insert PIN or insert incorrectly?
 
You can safely ignore the error if you are not facing any issues, it's just showing developer information about which component of the PBX generated the error. Yes it can be generated when the PIN is wrong like my example below. I tried to call 17777777777 without calling the PIN IVR first and this happens:
