Forwarding to cfd app broken in 16.0.7

Had this setup and working but now it is not working in the latest version. What should happen is that the call is forwarded to a call flow designer app extension that connects to a remote database and retrieves some text to be announced to the caller. Any suggestions on why using application name instead of extension number is working and how we can fix? Thank you.

you need to update the CFD builder, while rebuilding the project you will be asked to choose an extension number.

Then, you select "forward to extension" and choose that number for the CFD app.

Thank you. We have updated the CFD program and rebuilt the app. However we still cannot select the extension we chose we creating the app.
