There must be something I'm missing.

I'm reading this
https://www.3cx.com/docs/recommended-hardware-specifications-for-3cx/

For a medium on premise install (up to 50 extensions) recommends a 4VCPU and 4GB RAM i5.

But for cloud, recomends for medium (up to 50 extensions) amazon lightsail m5ad.xlarge which is 4VCPU 16GB RAM. I have not been able to find by m5ad.xlarge name in lightsail (maybe on deployment it appears) but anyway a 4VCPU/16 GB in lightsail shows me a prize of 80USD month. Is that what is needed? It is worst if I look for it on EC2 as that m5ad.xlarge instance is 0,23 hour which makes 165USD on 30 days months.

Why on cloud we need 4 times RAM of what's recommended on premise?

Then when you look for the 3CX template on EC2 marketplace it recommends t2.micro which is free first year. What for? Taking into account the recommendations on the provided link for 10 extension is a1.medium what exactly can be done with a t2.micro?. Again I can't be sure what is exactly a1.medium on lightsail. On EC2 is a 1VCPU 2GB so taking into account on premise recommends 1VCPU 1GB then it must be that one. On lightsail would be a 10USD/month and on EC2 that instance 20USD/month. The B1MS recommended for Azure shows only 4GB of temporary disk space (not sure what that means).

Can someone clarify if those recommendation on cloud are correct or not and share his recommendations?

On OVH i've used recently a VM should be good enough for your use, 12.99€ a month

Google is great as a hosted provider as they will actually inform you if your specified instance is under/over capacity as far as resources are concerned - a real money saver here!
 
The whole point of the cloud is that you can scale up or down with ease. I'd stick to the cloud equivalent of the x86 hardware and just pay attention to your resources.
