Inbound Caller ID Incorrect

Hi all - we are running v14 SP3 and Yealink T46G v 28.80.0.95 phones. The receptionist has noticed multiple times of instances where an incoming call will show as if coming from internal ext. (i.e John Smith: Wireless Caller) even when the call came through to our main number. It doesn't always happen, and I can't seem to recreate or think where the mix-up is occurring.
 
It may depend on how the call routes before being answered (forwarded?), or if the number matches one in the 3CX Phone book, or one (a directory),internal to the set. You may need to see if there is some sort of pattern...the same number is affected? A check of the 3CX log, for one of these calls , shortly after it happens, may provide some insight.
 
There is actually a chance the Provider is actually sending it that way for some reason, but as leejor pointed out, you could probably see that from the Logs if you first set them to Verbose.
