Internal call or external call

Which of these calls would be considered as an internal call - from an extension in the LAN to an extension in the WAN (remote teleworker)?

Calls can not be made to remote teleworkers at all, all extensions need to be in the same LAN.
Calls to external extensions connected via STUN
Calls made via a VoIP provider
Calls to external numbers via a PSTN Gateway

I'm not clear on what you are asking as there seem to be several questions, and a statement.

Any extension to extension call could be considered internal depending on your definition. In other words, it isn't using a trunk to router the call, either via a provider, or a bridge trunk.

I would certainly consider a call via a VoIP provider or PSTN gateway as external.

Calls are not connected via STUN. STUN is simply a tool that can be used to determine the type of network a device is "behind".
