I suspect I am missing something minor or this is just not a supported configuration.

Client has 3 small businesses, each has a separate DID/mailbox 555-888-1234 -> ext 500 and 777-333-4321 to ext 600
Both extensions have the DID assigned to their respective extensions with a voicemail configuration
The physical phone device, has the two lines added to the BLF as BLF monitoring
The extensions have a forwarding rule * to forward to the physical phone with extension 200

What works:
Any calls coming into the two DID's are routing to the physical phone correctly, based on the forwarding rules of the extensions

What doesn't work:
When the call goes unanswered, it dumps into the physical phones extension (200) voicemail instead of falling back to the originating extensions voicemail

The solution desired:
User wants to receive calls from multiple DID's routing to his phone and absent his answering, have those calls fall back to voicemail for the originating extension

Added perk:
When the incoming DID to extension rings to the physical phone, having the extension name displayed to allow him to know which line it is coming in from

Because the call has been forwarded to another extension, it is that extensions voicemail that the call will be sent to if unanswered. The call no longer "belongs" to the extension that did the forward.

What might work for you, is to create a ring group with one member, Ext 200. When they forward ,send the call to the ring group. Set the ring group destination if no answer as the originating extensions voicemail. It just depends on how many extensions they forward calls to.
