Normal conference - Call Failed

When in a call and I manually try dialing to a second number to conference them in, as soon as I hit the arrow after entering the phone number I receive the error "Call Failed" without any other explanation. The management console log simply shows "not answered", but like I said, the call immediately goes to "Call Failed".

Not sure where to go from here.

What model of phone ?

Sorry that would have been a good bit of info! Softphone. Same thing happens via windows client, web client, or iOS client. Each are remote. All other inbound/outbound calling works fine, and has for some number of years.

@northcide try to create a conference using only internal extensions, don't involve external numbers and let us know if there is any difference
