Phantom Voicemail message showing in Web Client Only

I have one extension that shows a phantom voicemail only in the Web Client when that voicemail does not exist.

The phone shows/states no voicemails, new, old or otherwise.
The Windows client shows no voicemails.
The Voicemail folder ( C:\ProgramData\3CX\Instance1\Data\Ivr\Voicemail\Extensions\xxx ) is completely empty.
Trying to delete the voicemail from the Web Client does literally nothing.
Trying to download the voicemail simply says "file not found", as it should since the extension folder is empty.
Restarting the 3CX services does not fix the issue.
Restarting the entire PBX does not fix the issue.

This is driving my end user batty. Thoughts?

PBX Version: 16.0.493 running on Windows 10.
 
Try pressing F12 in Chrome. Then while the console is open, right click your browser's refresh button.
Click on the below highlighted button, and then F12 to close console

If it still persists after this step, try logging into the extension from an Incognito window (to ensure nothing gets stored).

As a very last step, restart all the PBX services in case none of the above has resolved it.
 
Hard Reload, Incognito, different browsers, different computers - none have any effect.

Restarting the PBX services had no effect.
Shutting off the virtual machine the PBX runs on and restarting it had no effect.
 
Make sure that you login to the server with an administrator account. Most probably the XML file that refers to the source audio file is there but not visible.
 
I have logged into the server with the Admin account. The file just isn't there.
