Placing a Call on Hold will ring all extensions

Grandstream 2170 handsets, 3 phone all in same default ring group. When any call is placed on hold all extensions ring, not sure if this is an issue with the handsets or 3CX. Find no setting that seems related to this issue in either 3CX nor 2170 GUI

Exactly how are you implementing the hold? Have you checked the 3CX Activity Log (Verbose) to see if it is of any help?
